Reproducibility of late gadolidium enhancement of atrial ablation scar

چکیده
Methods Eleven patients with atrial fibrillation (AF, 4 persistent) underwent first time pulmonary vein isolation procedure. 3D LGE magnetic resonance imaging (1.5T Philips Ingenia) was performed at 3 months (91 ± 15 days) post ablation (scan 1), with a reproducibility scan three days later (scan 2). 3D LGE acquisitions were performed at 10, 20 and 30 min post gadolinium (Gd, 0.2 mmol/kg) (spatial resolution 1.3 × 1.3 × 4 mm, reconstructed to 0.6 × 0.6 × 2 mm). Scan 2 was performed 48-96 hours after the Scan 1. The left atrium (LA) was manually segmented and LGE enhancement projected onto the shell (maximum intensity projection technique). Each pulmonary vein (PV) was analysed by 4 independent observers. The percentage of scar encirclement of each PV was quantified visually between 0% (no scar) and 100% (complete encirclement of vein by scar). The LA shell was thresholded at 3.3 standard deviations above the blood pool mean.

A multi-center trial of LGE-MRI of the left atrium
Background Atrial fibrillation (AF) is the most common rhythm disturbance. Radio-frequency ablation (RFA) of the left atrium (LA) is effective for drug refractory AF patients. Outcome of RFA procedure depends on the degree of pre-ablation LA fibrosis and amount of post-ablation scar. Late gadolinium enhancement (LGE) imaging can detect fibrosis and visualize scar. However, only few centers with...
متن کاملThe reproducibility of late gadolinium enhancement cardiovascular magnetic resonance imaging of post-ablation atrial scar: a cross-over study
BACKGROUND Cardiovascular magnetic resonance (CMR) imaging has been used to visualise post-ablation atrial scar (PAAS), generally employing a three-dimensional (3D) late gadolinium enhancement (LGE) technique. However the reproducibility of PAAS imaging has not been determined. This cross-over study is the first to investigate the reproducibility of the technique, crucial for both future resear...
متن کاملDual-IR late gadolinium enhancement achieves better blood suppression than traditional IR in a swine model of atrial radiofrequency ablation scar
Background A good correlation has been found between the extent of late gadolinium enhancement (LGE) and clinical outcome in patients who have undergone radiofrequency (RF) ablation for atrial fibrillation (AF). Residual blood signal with the traditional inversion-recovery (IR) sequence often hampers scar visualization in these patients and causes poor repeatability of scar size measurements. T...
متن کاملDetection of left atrial scar and changes of cardiac function in relation to AF ablation, by 3D late gadolinium enhancement
Background Atrial Fibrillation (AF) is the most common arrhythmia in clinical practice; catheter ablation (AFCA) is a widespread treatment. Determinants of AFCA success and post-AFCA left atrial remodeling are poorly understood. Delayed Enhancement Cardiac Magnetic Resonance (DE-CMR) has been used to image LA scar post-AFCA and its relation to ablation outcome. Here we explored the role of post...
متن کاملDark regions of no-reflow on late gadolinium enhancement magnetic resonance imaging result in scar formation after atrial fibrillation ablation.
OBJECTIVES The aim of this study was to assess acute ablation injuries seen on late gadolinium enhancement (LGE) magnetic resonance imaging (MRI) immediately post-ablation (IPA) and the association with permanent scar 3 months post-ablation (3moPA). BACKGROUND Success rates for atrial fibrillation catheter ablation vary significantly, in part because of limited information about the location,...
